When is a GCM (Geriatric Care Manager) Required?

Families served by Senior Solutions Team typically require our GCM and/or Private Duty Care services due to one of the following situations:

		The senior lives at home and needs new caregiver support (4 hours per day to 24 hours per day).
		The aging loved one is in the hospital and needs assistance with planning for and accomplishing the transition home, and needs rehabilitation support upon arriving home to reach their previous level of functioning as soon as possible.
		The senior is living in Assisted Living or Board and Care home but has been having difficulties managing at this level of care.
		The aging individual wants comfort care only, or is transitioning onto a hospice program.
		The aging senior suffers from dementia or Alzheimer’s and is demonstrating new behaviors that are creating challenges for safe care at home.
		The loved one is hospitalized and requires additional care support due to the severity of medical symptoms or the need for one-on-one care for their health and safety.
		Your aging loved one does not have family support nearby.
