Our GCM Process
To reach the highest standard of living for your loved one requires an intimate knowledge of the issues facing the entire family. That’s why our approach is careful and comprehensive, yet extremely efficient and cost-effective.
We begin our process by conducting a detailed assessment to best identify the challenges your aging loved one and your family face. Based on our findings, we provide meaningful, realistic solutions, as well as helpful connections to resources in your community. All the while, our GCM serves as your single point of contact, guiding and supporting your family to a new level of care, health and contentment.
Step by step, the process looks like this:
Step 1: Our GCM meets with the senior and/or their family at their current location (home, assisted living, board and care, skilled nursing facility, or acute care hospital).
Step 2: Our GCM collaborates with the aging loved one, their family, their primary care physician, and any other appropriate parties that may be helpful in providing useful information for the purpose of establishing a comprehensive list of the loved one’s needs.
Step 3: Once the senior’s needs are established, our GCM educates them and their family regarding their choices for care and/or assistance, as well as the associated costs.
Step 4: Together, the GCM and family formulate a comprehensive plan that will result in the highest quality of life possible for the loved one, fully taking into account any financial or unique challenges.
Step 5: Our GCM’s remain available to help the loved one and family with as little or as much assistance as they feel they need to effectively execute the plan.
Step 6: The family begins to enjoy greater peace of mind and healthier lifestyle.
