How Does a GCM Help?
The primary goal of an experienced Geriatric Care Manager is to restore a high quality of life and peace of mind to the entire family. To achieve this, our GCM’s partner closely with any fellow healthcare provider involved in the care of your loved one, including primary care physicians, gero-psychiatrists, skilled home health care agencies, skilled nursing facilities, Assisted Living, Board & Care facilities, hospital staff, and family members assisting in care. We help ensure that the communication related to the senior’s well-being and plan of care is communicated to all appropriate parties and family members.
Our GCM’s are available to family members 24 hours a day, 7 days per week for urgent care needs. In essence, we become the direct line of communication in challenging times. If your aging loved one experiences a medical change of condition, or a traumatic event such as a fall, our GCM’s are available to accompany them to the hospital and provide vital medical history to the emergency team. We act as a liaison to family at a distance, and stay with your loved one to comfort and support them until you arrive, providing updates as the situation progresses.
