Senior Solution Team

A partnership built on care

Why Choose Us?

Our unique geriatric management team has expanded expertise in three vital areas of senior care management.

The Senior Solutions Team Story

Eight years ago in a skilled nursing facility not so far away, our current team of Geriatric Care Managers (GCM’s) began working together to serve the seniors in our community.  Sharing a similar passion for enhancing the quality of life for aging adults and restoring balance and peace of mind to their families, we started Senior Solutions Team in 2005 with the mission of helping even more families in ever more profound ways.

It was an ambitious goal, but thanks to our more than 75 years of combined experience in assisting seniors and their families and our comprehensive clinical backgrounds in nursing, therapy, and social work, we feel we have made a positive, lasting difference in hundreds of people’s lives.

We also believe that with every new success story, we gain new insights into the challenges aging presents to not only the senior but their entire family. Being seen as “one of the family” to so many of our clients, we empathize with them and understand the incredible stress imposed by the tough and new choices they have to make. And that’s exactly why we founded Senior Solutions Team in the first place – we love to help.

Now, over four years later, as a seasoned team we are superbly equipped to deal with any situation, whether it’s a chronic condition, assisting with physical therapy, or problematic behaviors related to Alzheimer’s or other dementias. Our diversity and experience also allows us to tailor our care to each client’s needs and budgets, which was our goal from the start.

Senior Solutions Team has built the model of senior care we envisioned those many years ago, one built on helping families with compassion, integrity and accountability. We’re proud of what we do and you will be too.

Amy excels in educating seniors, families and caregivers on how to effectively manage medical issues at home and create the highest quality of life possible.

Toward this end, Amy assesses seniors’ medical histories; provides medication oversight; assists in solving complex problems through collaboration with the senior’s physicians and other healthcare providers; acts as a liaison and advocate for the aging loved one during periods of hospitalization or crisis; and provides caregiver training for seniors with more complex medical needs, such as gastric-tube feedings, colostomy care, wound care, and special diets.

Our In-Home Caregivers

Because our in-home caregivers are the ones with whom your loved one spends the most time, it’s important to us that they meet the very highest of standards. That’s why they are put through an extensive screening process and are chosen for their dependability, experience and affinity for elderly care.

All our personal caregivers are passionate about their vocation. We understand that our business is people, and whether the caregiver is required for four hours a day or around the clock, they must not only be an attendant but a companion to your loved one, building a lasting relationship of trust and comfort. Relationships and personalities matter.

Just as important, our caregivers must be educated, experienced and well-referenced, which is why we hire not only companions, but Certified Nursing Assistants (CNAs) and Certified Home Health Aides (CHHAs). We know that they are our GCMs’ eyes and ears and accurate, continual communication ensures that your loved one gets the care and quality of life they deserve. This highly coordinated and personal attention also results in the entire family enjoying a greater peace of mind.

Finally, all our caregivers are bonded and insured and screened for criminal records, DMV violations and references and pass TB and drug tests.

In short, they aspire to be trusted family members.
